Dosage of Czocain Gel

Czocain Gel is a Prescription-Only Medicine (POM). Take as directed by the pharmacist or physician. The dosage is highly dependent on the area of application, the patient's size, and the procedure being performed. The lowest effective dose should be used to avoid high plasma levels and serious adverse effects.

Proper Administration

• Administration: Topical application to mucous membranes or internal surfaces. It is ineffective when applied to intact skin.
• Procedure-Related Use: For urethral application, the gel is instilled directly into the urethra via the nozzle. For intubation, it is applied thinly to the end of the tube.
• Missed Dose Guidance: This medication is typically used on an "as needed" basis for procedures; thus, missed dose guidance does not usually apply.

Contraindications of Czocain Gel

Do NOT use Czocain Gel if the patient:

• Drug Allergies: Has a known hypersensitivity or allergy to Lidocaine Hydrochloride or any other amide-type local anesthetic.
• Pre-existing Conditions: Has pre-existing severe conditions that can increase the risk of systemic toxicity, such as severe heart block, severe shock, or severe trauma/sepsis near the application site.
• Age Restrictions: Should be used with extreme caution in children, particularly infants under 3 years of age.
• Pregnancy & Breastfeeding: Use with caution. Consult a doctor before use, as lidocaine is excreted in breast milk. Use should only be considered if the expected benefit outweighs the potential risk.

Side Effects of Czocain Gel

Side effects are usually limited to the application site when used correctly. Serious side effects typically occur due to excessive systemic absorption.

• Common Side Effects (Mild and Expected): Temporary local skin reactions at the application site, including mild irritation, redness, pallor (paleness), swelling, or a burning sensation.
• Serious but Rare Side Effects (Requires Medical Attention):
  • Systemic Toxicity (CNS/Cardio): Dizziness, drowsiness, confusion, blurred or double vision, ringing in the ears (tinnitus), nervousness, tremors, seizures, slow or irregular heartbeat, or hypotension (low blood pressure).
  • Methemoglobinemia (Blood Disorder): Pale, gray, or blue-colored skin, lips, or nails; shortness of breath, headache, or unusual tiredness. This is rare but serious.
  • Allergic Reactions: Severe rash, hives, difficulty breathing, or swelling of the face, lips, tongue, or throat.
• What to do if side effects occur: If any signs of serious systemic toxicity or an allergic reaction occur, stop application immediately and seek emergency medical attention.

Precautions and Warnings of Czocain Gel

• Before Starting Czocain Gel: Inform your doctor if the patient has underlying health issues, including severe liver disease, heart problems, or a history of a blood disorder like methemoglobinemia or G6PD deficiency.
• During Use: Avoid application to large, open wounds, broken skin, or burned areas unless specifically instructed by a physician, as this increases systemic absorption. Avoid applying heat to the area after use, as this can also increase absorption.
• Drug Interactions: Inform your doctor of all other medications, especially those known to induce methemoglobinemia (e.g., certain nitrates, sulfonamides) or other local anesthetics, as concurrent use increases risk.
• Cerebral Malaria: High doses of topical lidocaine have been associated with CNS excitation.
